RESUMO

To assess the reliability, validity and responsiveness of the Chinese version of the atopic dermatitis control tool (ADCT). After this study obtained authorization for the Chinese version of the ADCT scale. 114 patients with atopic dermatitis were enrolled from the Department of Dermatology, Peking University First Hospital using convenience sampling from October 2022. Patients were surveyed using the General Information Questionnaire, Chinese version of ADCT, patient-oriented eczema measure (POEM),peak pruritus numerical rating scale (PP-NRS),dermatology life quality index (DLQI) and the global patient self-assessment for disease severity. Mann-Whitney rank sum test and Spearman correlation analysis were used for item analysis; content validity was assessed using content validity index (CVI); exploratory factor analysis was used to assess structural validity; Cronbach' alpha coefficient was used to assess internal consistency; Spearman correlation analysis was used to assess the correlation of ADCT with other scales to assess external responsiveness. The results showed that all items were retained by item analysis. I-CVI was 0.9-1, and S-CVI/Average was 0.983; the scale extracted one common factor by factor analysis, the cumulative variance explanation rate was 77.927%; the Cronbach' alpha coefficient of the scale was 0.937; the correlation coefficients of the Chinese version of ADCT with POEM, PP-NRS, and DLQI were 0.805, 0.861, and 0.709 respectively. In conclusion, the Chinese version of the ADCT has adequate reliability, validity and responsiveness, and is suitable for measuring disease control in Chinese patients with atopic dermatitis.

Ketone bodies have beneficial metabolic activities, and the induction of plasma ketone bodies is a health promotion strategy. Dietary supplementation of sodium butyrate (SB) is an effective approach in the induction of plasma ketone bodies. However, the cellular and molecular mechanisms are unknown. In this study, SB was found to enhance the catalytic activity of 3-hydroxy-3-methylglutaryl-CoA synthase 2 (HMGCS2), a rate-limiting enzyme in ketogenesis, to promote ketone body production in hepatocytes. SB administrated by gavage or intraperitoneal injection significantly induced blood ß-hydroxybutyrate (BHB) in mice. BHB production was induced in the primary hepatocytes by SB. Protein succinylation was altered by SB in the liver tissues with down-regulation in 58 proteins and up-regulation in 26 proteins in the proteomics analysis. However, the alteration was mostly observed in mitochondrial proteins with 41% down- and 65% up-regulation, respectively. Succinylation status of HMGCS2 protein was altered by a reduction at two sites (K221 and K358) without a change in the protein level. The SB effect was significantly reduced by a SIRT5 inhibitor and in Sirt5-KO mice. The data suggests that SB activated HMGCS2 through SIRT5-mediated desuccinylation for ketone body production by the liver. The effect was not associated with an elevation in NAD+/NADH ratio according to our metabolomics analysis. The data provide a novel molecular mechanism for SB activity in the induction of ketone body production.

OBJECTIVE@#To explore the clinical characteristics and genetic variants in a patient with adult ceroid lipofuscinosis neuronal type 7 (ACLN7).@*METHODS@#A female patient diagnosed with ACLN7 in Henan Provincial People's Hospital in June 2021 was selected as the study subject. Clinical data, auxiliary examination and result of genetic testing were retrospectively analyzed.@*RESULTS@#The patient, a 39-year-old female, has mainly presented progressive visual loss, epilepsy, cerebellar ataxia and mild cognitive decline. Neuroimaging analysis has revealed generalized brain atrophy, prominently cerebellum. Fundus photography has revealed retinitis pigmentosa. Ultrastructural skin examination has revealed granular lipofuscin deposits in the periglandular interstitial cells. Whole exome sequencing revealed that she has harbored compound heterozygous variants of the MSFD8 gene, namely c.1444C>T (p.R482*) and c.104G>A (p.R35Q). Among these, c.1444C>T (p.R482*) was a well established pathogenic variant, while c.104G>A (p.R35Q) was a missense variant unreported previously. Sanger sequencing confirmed that the daughter, son and elder brother of the proband have respectively carried heterozygous c.1444C>T (p.R482*), c.104G>A (p.R35Q), and c.104G>A (p.R35Q) variants of the same gene. The family has therefore fit with the autosomal recessive inheritance pattern of the CLN7.@*CONCLUSION@#Compared with previously reported cases, this patient has the latest onset of the disease with a non-lethal phenotype. Her clinical features have involved multiple systems. Cerebellar atrophy and fundus photography may be indicative of the diagnosis. The c.1444C>T (p.R482*) and c.104G>A (p.R35Q) compound heterozygous variants of the MFSD8 gene probably underlay the pathogenesis in this patient.

As arsenic widely exists in nature and has been used in the pharmaceutical preparations, the traditional Chinese medicine(TCM) with arsenic include realgar(As_2S_2 or As_4S_4), orpiment(As_2S_3), and white arsenic(As_2O_3). Among the above representative medicine, the TCM compound formulas with realgar are utilized extensively. Just in Chinese Pharmacopoeia(2020 edition), there are 37 Chinese patent medicines including realgar. The traditional element analysis focuses on the detection of the total amount of elements, which neglects the study on the speciation and valence of elements. The activity, toxicity, bioavailability, and metabolic pathways of arsenic in vivo are closely related to the existence of its form, and different forms of arsenic have different effects on organisms. Therefore, the study on the speciation and valence of arsenic is of great importance for arsenic-containing TCMs and their compound formulas. This paper reviewed four aspects of the speciation and valence of arsenic, including property, absorption and metabolism, toxicity, and analytical assay.

Objective To study the effect and mechanism of pearl hydrolysate on hepatic sinusoidal capillarization in liver fibrosis. Methods Hepatic sinusoidal endothelial cells (HSEC) and hepatic stellate cells (HSC-LX2) were incubated with Hepu pearl hydrolysate.The proliferation of HSEC and HSC-LX2 was examined by MTT colorimetry.The cell cycle and apoptosis of HSC-LX2 were measured by flow cytometry.The changes of the microstructures such as fenestra and basement membrane of HSEC were observed by transmission electron microscopy. Results The intervention with leptin increased the viability of HSC-LX2 (P=0.041),decreased the viability of HSEC (P=0.004),and caused capillarization signs such as decreased number and diameter of fenestrae and formation of continuous basement membrane.The treatment with pearl hydrolysate at different doses increased and expanded the fenestrae of HSEC (low dose:P=0.020;medium dose:P=0.028;high dose:P=0.032),disintegrated the extracellular basement membrane of HSEC (low dose:P=0.020;medium dose:P=0.028;high dose:P=0.032),decreased the viability of HSC-LX2 (low dose:P=0.018;medium dose:P=0.013;high dose:P=0.009),and induced the apoptosis of HSC-LX2 (low dose:P=0.012;medium dose:P=0.006;high dose:P=0.005).Pearl hydrolysate exerted therapeutic effect on capillarization in a dose-dependent manner (low dose:P=0.020;medium dose:P=0.028;high dose:P=0.032).Moreover,high-dose pearl hydrolysate showed stronger effect on capillarization of hepatic sinuses than colchicine (P=0.034) and salvianolic acid B (P=0.038). Conclusion Hepu pearl hydrolysate can increase the viability of HSEC,restore the area of fenestrae,disintegrate the basement membrane,and decrease the viability and induce the apoptosis of HSC-LX2,demonstrating significant pharmacological effects on the capillarization of HSEC and HSC-LX2.

Objective:To preliminarily analyze the relationship between peripheral blood CD19 +CD27 +B cells, CD4 -CD8 -double-negative T cells, related cytokines and recurrence in patients with neuromyelitis optica spectrum disorders (NMOSD). Methods:A retrospective analysis was performed on the clinical data of 72 patients with NMOSD admitted to Henan Provincial People′s Hospital between January 2019 and January 2021. According to presence or absence of recurrence within 1 year after treatment, they were divided into non-recurrence group ( n=30) and recurrence group ( n=42). The data such as gender, age and score of Extended Disability Status Scale (EDSS) at admission were collected. The levels of serum triglyceride (TG), total cholesterol (CHO), high-density lipoprotein cholesterol (HDL-C), low-density lipoprotein cholesterol (LDL-C), apolipoprotein A (ApoA) 1 and apolipoprotein B (ApoB) were detected by full-automatic biochemical analyzer. The level of total protein in cerebrospinal fluid was detected by full-automatic programmed protein analyzer. The levels of immunoglobulin (Ig) G and IgM in cerebrospinal fluid were detected by immunoturbidimetry. The counts of peripheral blood CD19 +CD27 +B cells and CD4 -CD8 -double-negative T cells were detected by flow cytometry. The levels of serum interleukin (IL)-6, IL-10 and IL-2 were detected by enzyme-linked immunosorbent assay. Results:EDSS score, neutrophils, proportions of cases with positive aquaporin 4 (AQP4) antibody and autoimmune antibody in the recurrence group were significantly higher than those in the non-recurrence group (all P<0.05). There was no statistically significant difference in serum TG, HDL-C, LDH-C, ApoB, ApoA1, total protein in cerebrospinal fluid, IgG or IgM between the non-recurrence group and the recurrence group (all P>0.05). The proportions of CD19 +B cells, CD19 +CD27 +B cells and CD4 -CD8 -double-negative T cells in the recurrence group were (1.21±0.12)%, (1.61±0.17)% and (1.39±0.25)%, significantly higher than those in the non-recurrence group [(0.85±0.07)%, (1.25±0.12)%, (0.89±0.22)%, t=15.51, 3.89, 12.06, all P<0.05]. The counts of CD19 +B cells, CD19 +CD27 +B cells and CD4 -CD8 -double-negative T cells in the recurrence group were (289.50±17.64) ×10 6/L, (4.67±0.03) ×10 6/L and (64.78±6.53) ×10 6/L, significantly higher than those in the non-recurrence group [(254.56±15.34) ×10 6/L, (3.18±0.03) ×10 6/L, (47.82±4.83) ×10 6/L, t=14.27, 4.26, 12.06, all P<0.05]. The level of serum IL-10 in the recurrence group was lower than that in the non-recurrence group [(18.56±1.97) ng/ml vs (24.72±2.52) ng/ml, t=11.64, P<0.05], while levels of IL-6 and IL-2 were significantly higher than those in the non-recurrence group [(15.12±1.54) pg/ml vs (11.47±1.23) pg/ml, (28.34±2.94) pg/ml vs (22.57±2.36) pg/ml, t=10.75, 8.89, both P<0.05]. Conclusion:The levels of peripheral blood CD19 +CD27 +B cells, CD4 -CD8 -double-negative T cells and related cytokines are abnormal in NMOSD patients, which may be related to the recurrence of NMOSD.

The incidence of psoriasis which is characterized by dryness, scaling and itching of the skin has been on the rise. It can have a profound psychological impact on patients' quality of life. Accumulating research has been conducted on the mechanisms of psoriasis in western medicine, from the difference of pathological manifestations of terminal keratinocytes, the disorder of expression of related factors and cells, to the immune imbalance of T lymphocytes and their subsets, or the abnormal transcription dominated by genetic genes. As a result, a complex and huge mechanism network has formed and many hypotheses have emerged. In recent years, with the in-depth research on non-coding genes, it has been clarified that microRNA-modified multi-pathway effect interferes with the occurrence and development of psoriasis and affects the proliferation, apoptosis, and differentiation of keratinocytes. The research on microRNA involves both genetics and immunology, which can help improve the key links in the micro pathway of psoriasis. Thus, it is a key part in the pathogenesis of psoriasis and has also become the hotspot and difficulty of modern research on psoriasis. At the same time, we should give full play to the advantages of traditional Chinese medicine (TCM) in syndrome differentiation and treatment. To be specific, microRNA targets of compound Chinese medicine preparations with the functions of clearing heat, cooling blood, detoxifying and removing blood stasis or effective medicinal monomers such as paeonol, tripterygium glycosides, shikonin, curcumin, total glucosides of paeony and indirubin should be explored, and microRNA can be used as the basis for blood syndrome differentiation of psoriasis. Thereby, the syndrome differentiation theory of TCM and micro indicators of western medicine are integrated, to make full use of characteristics of TCM and guide the clinical syndrome differentiation and treatment. At present, the intervention on microRNA in TCM is rarely studied, and available studies mainly focus on several targets such as microRNA-155, microRNA-210, microRNA-21, microRNA-203, microRNA-320, microRNA-124, microRNA-330, microRNA-146a, and microRNA-15a-5p. This paper summarizes the research on compound Chinese medicine prescriptions and monomers in the treatment and syndrome differentiation of psoriasis through the intervention of microRNA, which is expected to provide a reference for the research on psoriasis in TCM and western medicine and the establishment of microRNA-based database.

Follicular helper T (Tfh) cells are a newly discovered subset of CD4+ T cells. As reported, abnormalities in their development, differentiation, and function are closely related to the occurrence of autoimmune diseases. Psoriasis is an autoimmune skin disease and it is intractable with a prolonged course. At present, it is generally believed that immune imbalance mediated by T cells is the core mechanism of the pathogenesis of psoriasis. In the context of this mechanism, Tfh cells are associated with psoriasis, and their cellular level and abnormal expression of related candidates can promote the occurrence of psoriasis. In terms of treatment, Chinese medicine, by virtue of the characteristics of wide application and low price, serves as a good complementary and alternative treatment option for psoriasis. As confirmed by previous findings, some active ingredients or preparations of Chinese medicine used in the treatment of psoriasis can also intervene in and regulate the immune response mediated by Tfh cells and the related candidates. Based on the research reports and experimental data, the present study reviewed the research progress from the differentiation of Tfh cells, the relationship between Tfh cells and psoriasis, and the intervention and regulation of Tfh cells and related molecules by Chinese medicine, which is expected to provide certain theoretical support and references for the determination of new strategies for psoriasis treatment and research in related fields.

Objective:To investigate the changes of intestinal flora and serum amino acid levels in elderly patients with Parkinson's disease(PD)at different stages, and to provide evidence for diagnosis, prevention and treatment of PD.Methods:A total of 140 elderly PD patients diagnosed and treated in our hospital from January 2018 to August 2019 were enrolled and divided into PD course <5 years group(n=70)and PD course≥5 years group(n=70). Ninety subjects without PD confirmed by medical examination in our hospital were included as the control group.Real-time fluorescence quantitative PCR of 16S rDNA gene was used to detect levels of Bifidobacterium, Lactobacillus, Clostridium prasus, Enterococcus, Enterobacter, Prevotella copri and Akkermansia muciniphila bacteria in the gut, and the high-performance liquid chromatography was used to detect the serum amino acids in the three groups.Results:Compared with the control group, the levels of Bifidobacterium, Lactobacillus, Clostridium prasalis and Prevotella copri in the intestines were decreased and Enterobacter level was increased in PD course <5 years group and PD course≥5 year group( F=20.863, 32.251, 23.166, 24.683 and 10.136, all P<0.001). The serum levels of methionine, tryptophan, lysine and glutamate in PD course <5 years and PD course≥5 year group were decreased as compared with the control group( F=5.858, 5.877, 4.183 and 25.462, all P<0.05). Spearman correlation analysis showed that serum glutamate level was positively correlated with Clostridium prastia in the intestine( r=0.647, P=0.002)and negatively correlated with Prevotella copri( r=-0.559, P=0.010). Multivariate Logistic regression analysis showed that the protective factors in the intestines of elderly PD patients were Bifidobacterium( OR=0.186, 95% CI: 0.054-0.637, P=0.007), Lactobacillus( OR=0.283, 95% CI: 0.098-0.816, P=0.020), Clostridium prastia( OR=0.232, 95% CI: 0.063-0.851, P=0.028), and Prevotella copri( OR=0.222, 95% CI: 0.058-0.851, P=0.028), and the risk factor was Enterobacter( OR=5.119, 95% CI: 1.406-18.636, P=0.013). Conclusions:The decreases of Bifidobacterium, Lactobacillus, Clostridium, Prevotella copri and Enterobacter levels in the intestinal tract of PD patients, and the decrease of serum glutamate level may be correlated to the progression of PD.

Objective:To analyze the literature of Ilizarov methods, visualizing and discussing the research status, research hotspots and research trends.Methods:A bibliometric study on Ilizarov methods was performed. Using keyword "Ilizarov" and the Chinese translations to retrieve Chinese publications from CNKI database, Wanfang DATA, VIP database and SinoMed database. Using keywords "Ilizarov method, Ilizarov technique, Ilizarov treatment" to retrieve English publications from Web of Science Core Collection database before January 2021, extracting the information including author, journal, country, institution, keywords and cited times. Using the tools of bibliometrics and VOSviewer to analyze the data and draw knowledge maps.Results:A total of 1 789 Chinese publications and 1 709 English publications were included. USA, UK and China were on the top 3 list of the number of publications. Orthopaedic Journal of China, Chinese Journal of Orthopaedic Trauma, Chinese Journal of Reparative and Reconstructive Surgery, Chinese Journal of Orthopaedics, and Chinese Journal of Bone and Joint Surgery published the most literature. 7 English journals were identified as core journals in the field of Ilizarov methods, among which Clinical Orthopaedics and Related Research, Journal of Bone and Joint Surgery (British Volume), International Orthopaedics were JCR Q1 journals. According to the visualized keywords co-occurrence clusteranalysis, the research topics in Chinese literature could be sorted into 6 clusters, the latest hotspots were bone nonunion caused by infection and the treatment for the diabetic foot. In the English literature, the research topics could be sorted into 4 clusters, while the latest topics were infected and posttraumatic bone defects. Conclusion:The number of research on Ilizarov methods is increasing around the world, researches from China are gradually becoming one of the main forces. The research trends at home and at abroad are the same in essentials while differing in minor points, and researches with Chinese characteristics are developing. The strengthening of international cooperation is essential to the development of Ilizarov methods.

OBJECTIVE: To explore the effects of high temperature on learning and memory ability, behavioral activity, and fatigue as well as the intervention effect of compound nutrients on the exercising mice. METHODS: Thirty specific pathogen-free healthy male Kunming mice were randomly divided into the normal-temperature exercise, high-temperature exercise, and high-temperature exercise supplement groups, with 10 mice in each group. The mice in these three groups performed treadmill exercise for one hour every day, six days per week, and continued for four weeks. The mice in the high-temperature exercise supplement group were fed with 0.3 mL of compound nutrients 30 minutes before each treadmill exercise, whereas the mice in the normal-temperature exercise and the high-temperature exercise groups were fed with an equal volume of distilled water. At the end of the treadmill exercise, the mice were subjected to experiments on their neurological behaviors. The serum of mice in each group were collected to detect the lactic acid level, urea nitrogen level, and creatine kinase activity. The liver and gastrocnemius muscle tissues were then taken for detecting the levels of liver glycogen and muscle glycogen.RESULTS: Compared with the mice in the normal-temperature exercise group, the escape latency of the mice in the high-temperature exercise group was prolonged(P<0.05), whereas the number of platform crossings, percentage of target quadrant time, and distance were reduced(all P<0.05). Compared with the mice in the high-temperature exercise group, the escape latency of the mice in the high-temperature exercise supplement group was shortened(P<0.05), whereas the number of platform crossings, percentage of target quadrant time, and distance were increased(all P<0.05). Compared with the mice in the normal-temperature exercise group, the first fall time and grip strength of the mice in the high-temperature exercise group were reduced(all P<0.05), whereas the number of falls was increased(P<0.05). Compared with the mice in the high-temperature exercise group, the first fall time and grip strength of the mice in the high-temperature exercise supplement group were increased(all P<0.05), whereas the number of falls was reduced(P<0.05). Compared with the mice in the normal-temperature exercise group, the serum lactic acid level, urea nitrogen level, and creatine kinase activity of the mice in the high-temperature exercise group were increased(all P<0.05), whereas the levels of liver glycogen and muscle glycogen were decreased(all P<0.05). Compared with the mice in the high-temperature exercise group, the serum lactic acid level, urea nitrogen level, and creatine kinase activity of the mice in the high-temperature exercise supplement group were decreased(all P<0.05), whereas the levels of liver glycogen and muscle glycogen were increased(all P<0.05). CONCLUSION: High temperature exercise can lead to decreased learning and memory ability and behavioral activity in mice, resulting in exercise-induced fatigue. Supplemental compound nutrients can prevent these changes.

@#AIM: To evaluate the repeatability of a swept-source optical coherence tomography(SS-OCT)based biometer-IOL Master 700 in ocular biometrics in cataract eyes, and to investigate its agreement with a partially coherent optical interference based biometer-IOL Master 500. This study was also designed to compare the detection rate of the axial length(AL)measured by the two biometers. <p>METHODS:A total of 225 eyes of 133 cataract patients were included in this cross-sectional study from July to September 2018. Ocular biometric parameters including AL, keratometry(K), anterior chamber depth(ACD), lens thickness(LT), central corneal thickness(CCT)and white to white(WTW)were measured for each participant by both IOL Master 700 and IOL Master 500. <p>RESULTS: The IOL Master 700 measured <i>S<sub>w</sub></i> of AL, K<sub>m</sub>, K<sub>1</sub>, K<sub>2</sub>, ACD, LT, CCT, WTW in cataract patients were 0.027mm, 0.112D, 0.148D, 0.162D, 0.015mm, 0.010mm, 2.355μm and 0.134mm, <i>CoV</i> was 0.115%-1.162%, <i>ICC</i>≥0.923, the maximum <i>ICC</i> of AL is 1.000, and all measurements had high repeatability. The AL measured by IOL Master 700 and IOL Master 500 were(23.805±2.162)mm and(23.782±2.162)mm respectively, the difference was statistically significant(<i>t</i>=33.645, <i>P</i><0.001). However, the 95% <i>LoA</i> was 0.003mm-0.043mm, and the AL measured by the two biometers was significantly correlated(<i>r</i>=1.000, <i>P</i><0.001)with good agreement. The AL detection rates were 93.8%(211/225)for IOL Master 700 and 87.6%(197/225)for IOL Master 500, the difference was statistically significant(χ<sup>2</sup>=5.147, <i>P</i>=0.023).<p>CONCLUSION: IOL Master 700 demonstrates excellent repeatability in ocular biometric measurements including AL, K<sub>m</sub>, K<sub>1</sub>, K<sub>2</sub>, ACD, LT, CCT and WTW for cataract patients, especially in AL measurement. IOL Master 700 is in good agreement with IOL Master 500 in measuring AL, however the IOL Master 700 has a higher detection rate for AL in patients with severe opacity and subcapsular opacity in their lens.

RESUMO

MicroRNA (miRNA) is a type of small non-coding RNA and acts as a post-transcriptional regulator of gene expression. This article briefly describes the etiology of various chronic liver diseases, including metabolic dysfunction-associated fatty liver disease, chronic hepatitis B, chronic hepatitis C, chronic drug-induced liver injury, liver cirrhosis, and hepatocellular carcinoma, and summarizes related reports on microRNA-125b which enters different signal transduction pathways and plays the same or contradictory regulatory role in the same liver disease or pathological process by targeting different target genes, so as to provide insights into the research on the pathogenesis of various chronic liver diseases and the establishment of non-invasive differential methods.

@#In recent years, ocular biometers based on swept-source optical coherence tomography(SS-OCT)technology have emerged in ophthalmic practice. Compared with the earlier ocular biometers such as the partially coherent interference(PCI)based ones, the SS-OCT based devices have a higher signal-to-noise ratio, greater swept light wavelength, better tissue penetration, faster scanning speed, and better axial length(AL)detection rate for cataract patients. This review aims to summarize the advances of a widely used SS-OCT based device-IOL Master 700 in cataract.

@#AIM: To evaluate the repeatability and reproducibility of ocular biological measurements by a swept-source optical coherence tomography biometer IOL Master 700 in healthy eyes. <p>METHODS: A diagnostic test study of 103 healthy subjects was implemented. The right eyes were selected for measurements. Ocular biological parameters including axial length(AL), anterior chamber depth(ACD), anterior aqueous depth(AQD), keratometry readings(K), central corneal thickness(CCT), lens thickness(LT), white-to-white corneal diameter(WTW)and pupil diameter(PD)were measured using IOL Master 700 by two experienced examiners. One-way ANOVA was used to evaluate the repeatability and reproducibility for the measurements by IOL Master 700. <p>RESULTS: Regarding repeatability, the test-retest repeatability(TRT)for AL, mean K(Km), K1, K2, ACD, AQD, LT, CCT, WTW and PD obtained by the two examiners were 0.018(0.017)mm, 0.215(0.270)D, 0.325(0.334)D, 0.303(0.366)D, 0.049(0.041)mm, 0.048(0.042)mm, 0.058(0.047)mm, 6.168(5.779)μm, 0.395(0.406)mm, and 1.097(1.099)mm, respectively; the relative coefficient of variation(CoV)for all measured parameters but the PD were ≤1.219%; the intraclass correlation coefficient(ICC)for all measured parameters were ≥0.815. Regarding reproducibility, the TRT for AL, Km, K1, K2, ACD, AQD, LT, CCT, WTW, and PD obtained were 0.018 mm, 0.240 D, 0.329 D, 0.337 D, 0.048 mm, 0.048mm, 0.055mm, 6.396μm, 0.420mm, and 1.144mm, respectively; the ICC for all measured parameters was ≥0.900; the CoV for all measured parameters were ≤1.263% CoV, except for the PD(8.750%). In addition, the ICC of AL measurement for both repeatability and reproducibility were 1.000. <p>CONCLUSION: With the exception of pupil diameter, IOL Master 700 demonstrates excellent repeatability and reproducibility in ocular biological measurements for healthy eyes, especially in measurement of axial length.

Since the cluster of the 2019 novel coronavirus (2019-nCoV) pneumonia, a large number of patients gathered, the mortality of critical patients has remained high and the treatment was unclear. In this outbreak, Hunan Changde region immediately set up a hospital and intensive care unit. The patients relieved through respiratory support, hemodynamics management, nutritional support, the application of antiviral drugs, analgesic and sedation. The treatment experience in severe cases of 2019-nCov pneumonia patients were summarized as follows: in terms of respiratory support, we needed to pay attention to the advantages of high-flow nasal cannula oxygen therapy (HFNC) and the intervention of mechanical ventilation, pay attention to the ventilator parameters, and adopt prone position timely. In the aspects of fluid resuscitation and volume management, we should pay attention to the characteristics of severe patients' volume status, perform early evaluation, and clinicians should focused on hemodynamic management beside the bed. In the aspect of nutritional support and evaluation and maintenance of intestinal function, early enteral nutrition should be adopted in time. However, the trade-off between the risk of intestinal function and nutritional support in patients with mechanical ventilation and the antiviral benefits of Kaletra needed to be reevaluated, the optimized way of analgesia and sedation was adopted, at the same time, the usage and side effects of antiviral drugs should be paid attention to. We should grasp the opportunity of transportation for severe patients. It is suggested that some warning scores should be used to facilitate early recognition of patients with severe infection and then they should be earlier transferred to the designated hospital for intensive care.

Objective The purpose of this study was to investigate the mechanism of Bmi-1 regulating the sensitivity of non-small cell lung cancer (NSCLC) to chemotherapy by observing its effects on multidrug-resistance protein 1 (MDR1) and apoptosis-related proteins. Methods Small interfering RNAs (siRNA) targeting Bmi-1 were transfected into A549 and A549/DDP cells of NSCLC and the logarithmic-phase cells were randomly divided into a siRNA-Bmi-1, a siRNA-negative control and a blank control group. The A549 cells were treated with siRNA-Bmi-1, DDP or Bmi-1+DDP, or left untreated (the control). CCK8 assay was employed to measure the 50% inhibitory concentration (IC50) of cisplatin in the A549 and A549/DDP cells before and after treatment. The apoptosis of the cells was detected by flow cytometry, the mRNA expression of Bmi-1 determined by RT-PCR, and the relationship of Bmi-1 with MDR1 and cleaved caspase-3 proteins analyzed by Western blot. Results After transfection, the relative mRNA and protein expressions of Bmi-1 in the A549 and A549/DDP cells were significantly lower in the siRNA-Bmi-1 than in the blank control group (P < 0.05), but higher in the A549/DDP than in the A549 cells. The survival rates of the A549 and A549/DDP cells were decreased with the increased concentration of cisplatin (P < 0.05), even lower in the Bmi-1+DDP than in the DDP subgroup (P < 0.05). The apoptosis rate of the A549 cells was markedly higher in the Bmi-1+DDP than in the DDP, Bmi-1 and control groups ([39.65±3.41]% vs [23.11±1. 62] %, [2.05±1.56]% and [1.98±1.05]%, P < 0.05). After 24 hours of treatment with DDP, both the expressions of Bmi-1 and MDR1 were remarkably elevated, while the down-regulation of Bmi-1 significantly decreased the expression of MDR1 and increased that of cleaved caspase-3. Conclusion The expression of siRNA-Bmi-1 makes non-small lung cancer cells more sensitive to cisplatin, which might be associated with its inhibition of MDR1 expression and activation of apoptosis-related proteins.

Objective@#To evaluate the repeatability and reproducibility of swept-source optical coherence tomography biometrics (Tomey OA-2000) for measuring the biological parameters of the eyeballs in healthy people.@*Methods@#A diagnostic test was carried out in this study.One hundred and three healthy volunteers who were recuited from July to September 2018 in Affiliated Hospital of North Sichuan Medical College were included, and the right eyes were examined.Two examiners used OA-2000 for 3 consecutive measurements, respectively.One week later, the first examiner used OA-2000 for 3 consecutive measurements.The ocular biological parameters including axial length (AL), anterior chamber depth (ACD), corneal curvature (K), lens thickness (LT), central corneal thickness (CCT), pupil diameter (PD), and white to white (WTW) were measured.The repeatability and reproducibility of each parameter based on within-subject standard deviation (SW), repeatability limit of standard deviation (2.77SW), coefficient of variation (CoV) and intra-class correlation coefficient (ICC) was evaluated.This study followed the Declaration of Helsinki.This study protocol was approved by Ethic Committee of the Affiliated Hospital of North Sichuan Medical College (No.2018ER[A]019).@*Results@#Of the ocular biological parameters measured by OA-2000, CoV was less than 2% while ICC was greater than 0.950, except PD and WTW.The repeatability and reproducibility of K, AL and ACD were all good.For K1(ø=2.5 mm and 3.0 mm), the SW was less than 0.120 D, the 2.77SW was less than 0.350 D, the CoV was less than 0.293%, and the ICC was greater than 0.990; For K2 (ø = 2.5 mm and 3.0 mm) , the SW were less than 0.175 D, the 2.77SW were less than 0.485 D, the CoV was less than 0.396%, the ICC was greater than 0.980; for Km (ø = 2.5 mm and 3.0 mm), the SW were less than 0.099 D, 2.77SW were less than 0.275 D, CoV was less than 0.227%, ICC was greater than 0.996; ACD SW was less than 0.024 mm, 2.77SW was less than 0.066 mm, CoV was less than 0.668%, ICC were greater than 0.993; the SW of AL was less than 0.014 mm, the 2.77SW was less than 0.038 mm, the CoV was less than 0.057%, and all of the ICCs were 1.000.@*Conclusions@#Except PD and WTW, the ocular biological parameters measured by OA-2000 have good repeatability and reproducibility, in which the accuracy of AL measurement is the highest.
